What health form do I need?

Staff members

You will need a Class 3 Medical form, signed within 12 months of the last day you will be in Camp.

This includes all paid staff members, all CITs, all volunteer staff members, and all Commissioners, regardless of how much time you will be in Camp.

Campers under age 40

You will need a Class 1 & 2 Medical form (same form, weird numbering), signed within 36 months of the last day you will be in Camp. (These forms ONLY are good for 3 years.)

Note: if you will be participating in High Adventure activities (such as the Trekking program), you will need a Class 3 Medical form!

Campers over age 40

You will need a Class 3 Medical form, signed within 12 months of the last day you will be in Camp.
